“Based on staff interviews and records. The facility is open from 8:00AM-6:00PM. Director/Owner [name withheld] was away from the facility from 09/18/2025-10/15/2025. S(1) was not present on 09/24/25-09/26/25, 09/29/25,09/30/25 and 10/13/25. S(2) acting director worked from 8:45AM, 8:45AM-2:45PM on 09/24/25-09/26/25, 09/29/25,09/30/25 and from 8:45AM-2PM on 10/13/25. Leaving the facility without a director during the reamining hours of operation.”
