Violations
Text reproduced verbatim · California Department of Social Services, Community Care Licensing DivisionNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on record review and LPA observation of surveillance cameras, Staff #1 removed the infant from the chair, and abruptly pulled the bib off from the infant's neck upward over the child's head, startling the infant and causing the infant to wake up and cry. This poses a potential risk to the personal rights of children in care.”
“Based on interviews and LPA observation, on 05/30/2025, Staff was observed spanking Child once on the buttocks, lifting the child, turning the child around and sitting the child down. In addition to spanking a child, the same staff was also observed inappropriately handling Child in care by dragging the child by the feet and picking up Child from the ground by their wrists/forearm.”
“record review and interviews. The facility fed infant another infants breat milk. This poses a potential risk to the Health, Safety, or Personal Rights of children in care.”
“record review and interviews. The facility sent home incorrect infant bottle with another child name. This poses a potential risk to the Health, Safety, or Personal Rights of children in care.”
