“Ensure children are supervised at al times. LS observed four school age children leave the pre-k classroom to the lobby, exit the facility and get on a school bus while Staff #2 was inside the Pre-K classroom. A conversation was had with Staff #2.”

“Ensure staff are fingerprinted with in five days of turning 18. One staff was fingerprinted 16 days after turning 18.”

“Ensure all walls are in good repair. The computer room needs to have the walls repaired, room 111 needs two holes under the television repaired, and room 202 needs the chipped paint repaired.”

“Ensure their is no change in the quality of the air with air fresheners. Small room air fresheners were found in both men's restrooms, both women's restrooms, and the Pre-K classroom and classroom restroom.”

“OCCL received a complaint on August 17, 2022, regarding the club not following accident/injury requirements and not following child safety restraint regulations. An investigation was conducted on August 18, 2022. This complaint is substantiated because an accident/injury report was not given to the Reporter in a timely manner, and the car seat was improperly installed causing it to tip when the van turned a corner.”
