“Complete files are needed for the following staff members: Staff #1needs a service letter , Staff #2 needs two hours of annual health and safety training, Staff #3 needs one reference letter, employee declartation, alchol and substance abuse statement, a health apprasial with TB and DEED sertication, Staff #4 needs elibility letter and other documents for a cook.”

“Staff #5 needs blood lead screening results.”

“Ensure to use a foot activate pail for diapering and must be within arm reach of diaper table. Additionally ensure after diaper changing children hands are washed along with staff in the one- year - old classroom. ( This was corrected during the visit)”

“Provide a MAR for a diaper cream. ( This was corrected when the parent completed the MAR)”

“Ensure all feeding schedules are updated monthl;y for all infants in care.”

“Ensure to include weekly opporunites on the two year old and preschool lesson plans.”

“Ensure to submit a plan review when there is a change in licensed space. A wall was removed in the infant classroom to make the room bigger. Submit a plan review for approval to reflect this change.”

“Remove all hazards (nail, construction debris) from the outdoor play area.”

“Ensure direct supervision and observation of children is provided at all times. Ensure infants and toddlers are cared for in separate room from preschool and older. Two one-year-old and two two year-old year observed napping with preschoolers in the preschool classroom. During the visit the building only had three staff members. This was discussed with the staff members and they agreed. OCCL will conduct a follow up visit.”

“Ensure every exit, exit access and exit discharge is free of obstruction at all times. The exit to the back of the building is blocked by construction work.”

“Ensure to provide a quiet activity to children who are not resting and ensure to maintain staff to child ratio when children are not sleeping. One staff member was observed with 12 children the youngest was one year old. ( This was discussed with the staff member)”

“Submit and received approval before changing/ making additions to the indoor or outdoor area. LS observed construction work to center and outdoor play area has been relocated. Submit plan review to OCCL”
