Delaware publishes only visits where a regulation was cited, plus complaint investigations. Visits that found no violations do not appear in the state's data, so a gap between dates here does not mean the program went uninspected. Delaware does not publish a severity weight for its citations.

Violations
Text reproduced verbatim · Delaware Office of Child Care LicensingNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio, staff using alcohol or drugs that could adversely affect job functions, the administrator not being present the required time, lack of positive behavior supports, handwashing, and diapering procedures. An investigation began on February 18, 2026, and concluded on February 19, 2026. This complaint is substantiated because, according to the administrator s attendance record, Owner #1 was not present the required 27.5 hours per week. There is no evidence to substantiate the allegations of improper staff-to-child ratio, staff using alcohol or drugs that could adversely affect job functions, lack of positive behavior supports, handwashing, or diapering procedures based on interviews, observations, and a review of documentation. An additional concern noted during this investigation was children were observed lying on their sleeping equipment without top or bottom covers.”
“Ensure staff files are completed. Staff #1 needs an update health appraisal, Staff#2 needs an updated medication certification, Staff #3 needs an updated fingerprint and eligibility determined and Staff #4 needs eligibility determined.”
“Ensure the handwashing sink have running water. (This was corrected when the owner purchased and fixed the sink pipe so the water can run again)”
“Purchase additional supplies such as age-appropriate books for the two-year- old classroom.”
“An updated comprehensive background check and eligibility determined for Staff #1”
“Ensure child files are completed. Child #2 and Child #11 needs blood lead test result.”
“Updated radon test results are needed for the center.”
“Repair or replace the broken toilet.”
“Supply additional water for emergency use at the center.”
“Revise the lesson plan to include daily activities for the preschool classroom and the two year old classroom.”
“Complete files are needed for the following staff members: Staff #1 and Staff #2 needs the OCCL health and safety training”
“Add a weekly special activity to the lesson plan for the preschool classroom.”
“Complete files are needed for the following staff members: Staff #1 needs a health appraisal with TB, and an administration of medication certification. Staff #2 needs administration of medication certification”
“A foot activated diaper pail is needed in the bathroom for diapering. (corrected when a staff member place a diaper pail in the bathroom.)”
“Provide special activity on the lesson plan in the two-year-old classroom, (corrected during visit when the staff added the special activity on the lesson plan.”
“The Office of Child Care Licensing (OCCL) received a complaint on October 14, 2022, regarding improper staff-to-child ratios and security procedures. An investigation was conducted on October 21, 2022. This complaint is substantiated for not following security procedures because a staff member stated the center door is propped when they go outside to ensure re-entry into the building. There is no evidence to substantiate the allegation of improper staff-to-child ratio because proper staff-to-child ratios were observed during the visit, and attendance records from October 10, 2022, to October 14, 2022, reflected proper staff-to-child ratios. A concern noted was staff were not signed out on some attendance records.”
“OCCL received a complaint on July 14, 2022, regarding improper staff-to-child ratios. An investigation began on July 26, 2022, and concluded on August 8, 2022. This complaint is substantiated because according to attendance records, on July 1, 2022, July 5, 2022, July 6, 2022, July 7, 2022, July 8, 2022, and July 11, 2022, Staff #1, an aide, was alone with children in the one-year-old and two-year-old classroom. Also, during the visit, two staff members who are aides were alone with five children. Additional concerns noted were attendance records for staff and children were not accurately documented on several occasions.”
“Ensure direct supervision of children is maintained at all times in each classroom by a qualified staff. In the infant classroom an aide (BC) was left alone with three infants, while another staff (DS) ECT change an infant in another room locating in the back of the room. This was corrected when staff member TH (ECA) joined he classroom.”
“Complete files are needed for the following staff: Staff #1 needs administration of medication certification. Staff #2, 6,9,10 needs CPR/ first aid certification Staff #3 needs CPR/ first aid certification, annual safe sleep, annual child abuse and neglect recognition, four reference letters, a health appraisal with TB results. Staff #11 needs release of employment, service letter, Staff #12 needs health appraisal, employee declaration and annual child abuse and neglect recognition.”
“Health appraisals are needed for child #1 and Child #7”
“Ensure to document the whereabout of each child in care. In the three year old classroom, one child transition to the Pre-K classroom and another child transition to the two year old classroom. The attendance record reflected eight children in the classroom.”
“Ensure sleeping equipment are not touching each other. LS Donovan observed bedding stacked with sleeping equipment.”
“On October 5, 2021, a complaint was accepted for investigation by OCCL regarding lack of supervision and not ensuring non-prescription medication is stored securely out of reach of children. LS Donovan and McPhatter investigated the complaint from October 7, 2021, to October 13, 2021. This complaint is substantiated for storing non-prescription medication in the first aid fanny pack where a child gained access and swallowed an antacid. There is no evidence to substantiate this complaint for lack of supervision because the staff was present on the playground caring for an injured child while Child #1 ingested the antacid pill. Concerns noted were a child received an injury while on the playground and the center did not document the injury and the center failed to notify OCCL verbally within one business day a child receiving medical treatment due to an injury at the center.”
