Delaware publishes only visits where a regulation was cited, plus complaint investigations. Visits that found no violations do not appear in the state's data, so a gap between dates here does not mean the program went uninspected. Delaware does not publish a severity weight for its citations.

Violations
Text reproduced verbatim · Delaware Office of Child Care LicensingNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One enrolled and the provider's child were observed in the child care space while the provider was outside talking with a neighbor. [name withheld] was addressed and agreed to always have children in direct supervision without exception.”
“Accurate attendance with exact time in and out must be maintained. Two children were signed in and out for the day of the visit and had not attended. Send attendance sheets for the week of April 10 and 17 to OCCL on Friday, April 21, 2023.”
“Submit a health appraisal w/TB for the new substitute.”
“Submit health appraisals with immunizations for two children and a blood lead test for one child.”
“Assure each child is signed in and out with each arrival and departure.”
“Submit documentation to indicate replacement of the outlet cover in the bathroom.”
“Provide documentation of replacement of the torn nap mats and storage of the nap equipment not touching.”
“Add an instant cold pack to complete the first aid kit.”
“Submit a renewal application 60 days pror to license expiration.”
“Submit documentation of bottom coverings for the sleep mats.”
“On June 28 and 29, 2022, OCCL received two complaints regarding FCCH provider [name withheld] regarding lack of supervision. An investigation was initiated on June 30, 2022, by phone and an investigation visit was conducted on July 7, 2022. Based on the results of the investigation, this complaint is substantiated because [name withheld] left a three-year-old child unattended in the park for at least 45 minutes. [name withheld] left the park and did not notice the child was not with her. Other non-compliance items were also noted during the visit because attendance records were not complete, [name withheld] was over capacity by having four additional children, and [name withheld] provided conflicting information to the police officer who returned the child and to OCCL.”

“Attendance records for children must state exact times of arrival and departure and be recorded at those times. [name withheld] s attendance was not completed each day and no times were recorded. Provide July s attendance record to OCCL by Monday, August 1, 2022. Submit a written plan indicating how the provider intends to assure accuracy with attendance.”
“A level one FCCH may have up to five preschool-age children present or four preschool-age and two school-age children. [name withheld] had six preschool-age and younger children in care with four under 24 months of age. Adhere to the schedule for daytime and evening care created to correct the non-compliance in the July 18, 2022 complaint. Capacity may not be exceeded at any time for both the total number and age of the children. Send a revised schedule to OCCL by July 29, 2022.”
