How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

Substantiated complaint

“On October 1, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding release of children, security procedures, lack of supervision, and not using respectful communication. An investigation was conducted on October 2, 2026. This complaint is substantiated for not following the release of children procedures because although Administrator #1 and interviewed staff all stated Person #1 was known to the facility as an authorized individual for Child #1 after their identity and authorization were initially established, Administrator #1 failed to make and keep a copy of Person #1 s identification. There is no evidence to substantiate for security procedures because Administrator #1 and interviewed staff stated the rear entrance could not be independently accessed from outside when the door was closed and that individuals arriving while children were outside would encounter staff on the playground. LS #1 observed the rear doors of the facility remain secure and that the doors cannot be opened from the exterior once closed, even if unlocked from the interior. There is no evidence to substantiate for lack of supervision because both Administrator #1 and Staff #1 stated no staff member witnessed an incident resulting in the marks and that the marks were first brought to staff s attention during pickup. An incident report was subsequently completed. There is no evidence to substantiate for not using respectful communication because Owner #1 denied the allegations.”

“On April 22, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding not following the policies in the parent handbook, positive behavior supports, lack of supervision, the playground gate not working properly, and improper staff-to-child ratio. An investigation was conducted on April 23, 2026. The complaint is substantiated for the gate to the outdoor play area not working properly and improper staff-to-child ratio. The gate to the outdoor play area was observed not self-closing and not self-latching. The attendance record for April 24, 2026, showed the preschool classroom had up to 10 children, the youngest being two years old with one staff member from 8:48 am to 9:30 am. There is no evidence to substantiate the allegations of not following the policies in the parent handbook, positive behavior supports, and lack of supervision because staff denied the allegations.”

Substantiated complaint

“On April 22, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding the center not following their policies in the parent handbook, a lack of positive behavior support, and not providing a quiet activity for children after 30 minutes who do not want to sleep. An investigation was conducted on April 23, 2026. The complaint is substantiated for a lack of positive behavior supports because Owner #1 was heard on video footage speaking to Child #2 in a threatening manner for failing to fall asleep. The complaint is also substantiated due to non-compliance with the center s suspension policy outlined in the parent handbook. Documentation, including staff input and communication between Parent #1 and Administrator #1, confirms that Child #2 was subjected to a two-week suspension, which violates the established protocol of a one-day suspension followed by a five-day notice period for alternative care. Furthermore, the suggested removal of Child #2 to a smaller classroom contradicts the center s documented inclusion policy, which promotes the integration of children with varying needs in a shared environment. There is no evidence to substantiate not providing a quiet activity for children after 30 minutes who do not want to sleep because staff denied the allegations.”
