“Ensure proper staff-to-child ratios are maintained at all times. When the licensing specialist entered the Dolphins classroom, she observed one staff member with 17 four and five year olds. Shortly after, a second staff member entered the classroom. When asked, she stated she left the classroom to grab something from the classroom next door. This was discussed with the staff member and corrected when she agreed to comply in the future. Ensure proper supervision is provided at all times. According to a conversation with a staff member, preschool-age and school-age children are permitted to leave their classrooms to use the bathroom located in the hallway directly next to their classrooms without proper staff supervision. According to staff, a staff member stands in the classroom doorway to supervise children in the bathroom. This was discussed with staff and corrected when they agreed to comply in the future.”

“Ensure staff files are complete. One staff member needs an adult health appraisal. One staff member needs to complete an in-person skills demonstration for CPR.”

“Ensure child files are complete. Three children need blood lead screening results.”

“Ensure hazardous materials are stored inaccessible to children. Several paint cans were observed in an unlocked cabinet in the school-age Orcas classroom. Remove the paint cans or lock the cabinets.”

“Ensure all freezers are 0 degrees or colder. The thermometer in kitchen freezer read 10 degrees F.”

“Ensure the gates in the outdoor play area are self-closing and self-latching. One gate in the Pacific playground and one gate in the Atlantic playground are no longer self-closing and self-latching.”

“Ensure staff members document their exact hours they work in the classroom. Staff members did not sign in and out the classrooms. Provide staff members attendance for each classroom from 7-12-2023 to 7-19-2023.”

“Complete files are needed for five children. Updated feeding schedules are needed for five infants.”
