Source: Delaware Office of Child Care LicensingRecords held from Sep 27, 2021 onward. Earlier visits may exist in the agency’s own files.
Delaware publishes only visits where a regulation was cited, plus complaint investigations. Visits that found no violations do not appear in the state's data, so a gap between dates here does not mean the program went uninspected. Delaware does not publish a severity weight for its citations.

Violations
Text reproduced verbatim · Delaware Office of Child Care LicensingNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Ensure staff files are complete. Three staff need an adult health appraisal on OCCL's form. One staff member needs service letter attempts from four previous employers.”
“Ensure that each tour and family that enrolls signs the Parent's Right to Know Log. Three newly enrolled children were not on the log.”
“On August 13, 2025, the Office of Child Care Licensing received a complaint regarding screen time, not following the schedule, lack of positive behavior supports, smoking, and lack of supervision or staff-to-child ratio when the staff leaves the classroom. An investigation was conducted on August 25, 2025. The complaint is substantiated for staff-to-child ratio because Staff #2 and Staff #3 left Staff #4, an aide, alone in the gym with up to 38 school-age children. Staff #3 also left the gym to retrieve two children from the lobby, leaving 38 children in the gym with one qualified staff member and one aide. Staff #5 stated she had left the classroom, and it was out of ratio when she left. The complaint is substantiated for not following the schedule because all morning the children were in the gym and the only classroom that was supposed to be in the gym was the Scott classroom. There is no evidence to substantiate the allegation of not implementing positive behavior supports because Staff #5 stated what procedures are used when undesired behavior is exhibited and all the interviewed children stated they have not and have not seen their friends yelled at or yelling at the face. There is no evidence to substantiate the allegation of staff providing vapes to children or smoking on the premises during hours of operation because all staff and children interviewed denied seeing smoking or vapes being provided to children. There is no evidence to substantiate regarding screen time requirements because although the television was on during the visit, staff denied the allegation of watching television all day. LS #1 explained the screen time regulation, and when LS #1 returned to the gym, the television was turned off. Additional concerns were observed, including the maximum group size for school-age children being exceeded, with up to 45 children in the gym and unqualified staff alone with school-age children in the lobby and the gym. In addition, Staff #6 di d not have a complete file on-site.”
“On June 11, 2025, the Office of Child Care Licensing received a complaint regarding a lack of supervision, no permission for children to walk from the bus stop to the center alone, and a lack of positive behavior support. An investigation began on June 13, 2025, and concluded on June 16, 2025. The complaint is substantiated for a lack of supervision because Person #1 and Staff #4 stated there were no staff members present at the bus stop to meet the children. The complaint is substantiated for a lack of positive behavior support because staff stated Staff #4 told a child they would shove a pool stick up his a**. There is no evidence to substantiate the complaint for not having permission for children to walk from the bus stop to the center alone because although the children walked alone, it was a supervision issue and the facility s policy is the children are not allowed to walk from the bus stop without an adult.”
“Ensure parents and guardians are welcomed and permitted without prior approval. The Specialist observed a guardian sign in two children and then left them with the staff in the hallway. Another staff member retrieved the children and walked them to the classroom. All staff stated that parents are not permitted for security reasons.”
“Ensure staff files are complete. One staff member needs a health appraisal on OCCL's form, and three staff members need service letter attempts.”
“Ensure staff attendance is documented at exact arrival and departure. The specialist observed staff leaving and returning to the classroom but not signing out or in.”
“Ensure that classrooms two and three have posted schedules and activity plans.”
“Staff #1 was observed alone in the classroom with seven children This was corrected at the visit when the staff member was joined by another staff member.”
“Staff !1 needs a health appraisal, deeds certification, OCCL 7 hour health and safety training, and release of employment Staff #2 need CPR/FA certification, OCCL 7 hour health and safety training, a health appraisal and a service letter Staff #3 needs OCCL 7 hour health and safety training, a health appraisal w/TB and two reference Staff #4 needs a complete file Staff #5 needs a health appraisal w/TB”
“Provide OCCL with a copy of the Parents Right to Know Log”
“Child #7 needs a health appraisal”
“One child needs a MAR. Two were observed instructing to administer two different dosages.. Which one should be administered? Send copy of new MAR”

“The Office of Child Care Licensing received a complaint on December 8, 2023, regarding positive behavior supports, staff communication, accident injury procedures, and staff participating in other activities when caring for children. An investigation was conducted on December 27, 2023. This complaint is substantiated for staff communication because staff admitted to arguing in front of children. There is no evidence to support the allegations of lack of positive behavior supports, accident injury procedures, and staff participating in other activities because interviewed staff denied these allegations.”
“Ensure a valis insurance card is available in each van.”
“Ensure staff without qualifications submit within 60 days of hire. One staff member has been employed since June 1, 2022 and has not submitted for a Delaware First Credential.”

Ten staff need annual training hours, a professional development plan, and Health and Safety Training for Child Care Professionals. Eight staff need annual review of abuse and neglect and letter of eligibility on file. Two staff need employee declaration, drug and alcohol statements, a release of employment, and service letter attempts to prior employers with follow-ups.
