Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“During the inspection, it was determined the program did not protect children from an unsafe item, condition or equipment. The following was observed: extension cord(s); electrical cord(s) attached to an object that could result in a severe injury if pulled. Any hazardous equipment must be removed, replaced, or repaired and any hazardous condition must be corrected and must be made inaccessible to children. Provide staff training. Submit the program’s corrective action plan, which includes a statement that the item or condition has been removed and a statement that training was provided, to the department to verify compliance with the requirements of this rule.”
“During the inspection and review of staff files, it was determined medical statements for the individuals used to count in ratio listed on the Employee Record Chart did not meet the requirements in that a medical statement was not on file; a statement verifying the individual is free of communicable disease and fit to work with children was missing. Submit the program’s corrective action plan, which includes a copy of the medical statement, to the department to verify compliance with the requirements of this rule.”
