Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“During the inspection, it was determined that the outdoor play space was not free of hazards in that protruding bolts were observed on fencing, there was peeling paint on a door, and large openings on a storm drain cover. Submit the program's corrective action plan to the department to verify compliance with the requirements of this rule.”

“During the inspection, it was observed that furniture and equipment does not meet safety and sanitation requirements. Children had access to electrical cords attached to a television that could result in a severe injury if pulled.”
“During the inspection and review of staff files, individuals used in ratio did not have documentation of the required orientation training prescribed by the department completed within thirty days of the start date.”
