Violations
Text reproduced verbatim · Ohio Department of Children and YouthNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

“It was observed that a child was left unattended inside the facility when the floating teacher left a child in the restroom and returned to the classroom without the child. The child returned to the classroom on their own.”
“During the inspection, sufficient individuals were not physically present with children to meet staff/child ratio. A 2:18 ratio was observed for integrated classrooms during outdoor play when Room 160 and Room 165 were combined.”
“During the inspection, it was determined the following requirement was not met: • — Child’s information, medication name, or dosage information was missing *@ Medication was not in original container or the container from which it was dispensed • Medication was expired.”
“During the inspection and review of staff files, individuals used to count in ratio did not have evidence of a signed medical statement on or before the first day of employment.”
“During the inspection and review of staff files, at least one person onsite does not have a current age appropriate course in first aid from an approved provider. Rules in Compliance/Not Verified”
