How to read violations

Nearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

Paperwork — records, postings, forms Standard — supervision, ratio, training Health & safety — direct risk to a child

Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.

StandardState weight: Moderate risk
5180:2-12-18 — Ratio

“During the inspection, required staff/child ratios were not maintained for [the same group/different groups] on multiple occasions, as noted below: The ratio determined for the Older Toddler/Preschool group was: - one (1) Child Care Staff Member for nine (9) children. A second staff member entered soon after arrival. Additionally, a ratio of one (1) Child Care Staff Member for nine (9) children was determined for the Older Toddler/Preschool group when a staff member left the class to take a child to the restroom. Additional staff members must be hired or current Child Care Staff Members must be rescheduled to maintain compliance. Provide staff training. Submit the program’s corrective action plan, which includes a statement that training was provided, to the Department to verify compliance with the requirements of this rule.”
