Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Proof of education needed for three caregivers..”
“Children were observed moving between two classrooms freely. As a result, one child was able to leave the classroom and facility. Caregivers were unaware of her absence.”
“Based on the investigation findings, the standard was found to be noncompliant. A child was left unsupervised and was able to exit the facility and cross the street to a parking lot.”
“DFPS investigates and determines that a staff member at an operation is found Reason to Believe for abuse, neglect, or exploitation of a child in care. This was a result of a lack of supervision.”
“Based on the investigation findings, the standard was found to be noncompliant. A child's nap mat was placed behind a shelf. Resulting in the caregivers not having visual awareness of the child.”
“The center did not have verification of a current fire inspection.”
“The center did not have test data from the manufacturer showing the impact rating of the material (the maximum height of equipment that may be installed over the surfacing material), and installation and maintenance requirements.”
“The center did not have verification of a current gas inspection.”
