Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of 10 children files reviewed did not have a current immunization record on file.”
“One caregiver did not wash their hands after wiping a child's nose. Note: hands were washed when pointed out.”
“Two staff's files were missing the education requirement.”
“One staff present and had direct access to children did not have an active background check. Note: the staff member left during this inspection.”
“One child was left unsupervised in the bathroom for 15-20 minutes.”
“The incident report was not signed within 48 hours of the incident.”
“Two caregivers did not have the required annual training.”
“The playground maintenance checklist list was not available.”
“One caregiver did not supervised the classroom by stepping outside the classroom.”
“Four out of 14 staff's files reviewed were missing the 24 hours of annual training.”
“One caregiver in the 2 year old classroom was changing diapers and cleaning faces and did not wash her hands in between changing gloves. Note: The caregiver washed her hands during the inspection.”
