Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Lysol was observed in a classroom on a shelf that is accessible to children. Hard surface sanitizer was used to prop open the girl's restroom door. This was corrected when the items were moved out of reach of children.”
“Two infants were asleep with a bottle inside of their crib. This was corrected at inspection when the caregiver removed the bottles from each crib.”
“The trim in the boy's restroom was peeled off causing damage to the walls and paint chipping. A light fixture on the celing tile in the cafeteria was broken leaving the lights exposed.”
“The nail bolts in the black barrier around the playground were sticking out causing a risk to children. A train that is anchored on the playground also had nail bolts sicking out. This was corrected when a caregiver hammered the bolts in place.”
“A hose was observed in the play ground area where children play. This was corrected when the Director removed the hose.”
“A face plate to an electrical outlet was broken. This was located in an area that is accessible to the children. A picture was taken.”
“A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.”
“One of nine children records evaluated did not have a health statement available.”
“The first aid kit in the van did not have a pair of scissors. This was corrected at inspection.”
“One of nine children records evaluated did not have a signed medical consent to treat available.”
“One of nine children records evaluated did not have an emergency contact other than a parent available. This was corrected at inspection.”
