A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“The van used for transportation did not have a safety system alarm.”
“A child was left unsupervised in the classroom for approximately one minute when the class left the room for a fire drill. This was self reported. This was corrected at the time of the inspection when all caregivers were observed supervising in compliance with minimum standards. The caregiver was counseled and retrained on supervision and transitions.”
