Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operation was observed to not have activity plans for the toddler-two classroom available for review.”
“The operation was observed not have adequate walk space between each cot in the napping area at the time of inspection.”
“The operation was observed to have thirty children with a specified age group of two years in the napping area with two caregivers at the time of inspection.”
“The operation was observed to not have activity plans available for review.”
“There is no loose fill under the dome climbing structure.”
“There is no transportation for the director.”
“There was no emergency contact info for one out of four children's files.”
“There was no sanitation report on file at the center.”
“The kitchen appeared to be unsanitary with dead roaches in various states of decay on floors and shelves, grime in the corners of floors and surfaces, and a general unpleasant odor.”
“The owner stated that she has been performing her own pest control tasks, and a bag of DIY pest control items were observed. A conversation was had with the owner about DIY pest control being against minimum standards and she stated that she will no longer perform her own pest control tasks.”
“There was sufficient evidence to suggest that children are not given second servings.”
“The gas leak test had not been performed within the last two years.”
“he fire drills had been conducted according to the director, but had not been properly documented since March. This was corrected when the director conducted a fire drill and recorded it while I was present.”
“The carbon monoxide detectors had been tested, according to the director, but had not been properly documented since March. This was corrected when the director tested the alarms and recorded the results while I was present.”
“The fire extinguishers had not been documented as inspected since March. This was corrected when the director checked it and recorded her findings while I was present.”
