Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Six inches of loose surfacing needed on both of playground.”
“Two caregivers were found sleeping in a classroom while children are in her care.”
“Two of the ten children's records did not have up-to-date imunization record on file.”
“Based on the investigation findings, this standard was reviewed and found to be in non-compliance. The child was unsupervised for less than a minute when they wandered out of the classroom and into the hallway.”
“A child went to the bathroom that was located in the classroom. While in the bathroom the teacher and the remaining children left the room. This left the child alone in the room. The child was alone for less than two minutes.”
“The caregiver failed to demonstrate competency when she did not complete a head count before leaving the classroom. As a result, a child was left alone in a classroom.”
“10 out of 10 children's files were missing the emergency contact's addresses.”
“A caregiver was present, and the operation did not have caregiver background check result. This was corrected when the caregiver was sent home, during my inspection.”
“This standard was under review due to an investigation, and was found deficient when the operation notified licensing of an incident, where a two year old was left unsupervised, 4 days after the incident took place.”
“This standard was under review due to an investigation, and was found to be deficient. A caregiver was not properly supervising a child, when a two year old was able to walk out of the classroom and walk through the hallways.”
“This standard was under review due to an investigation, and was found to be deficient. The operation fail to notify the parent, the day of the incident, when a two year old walked out of the classroom and was unsupervised.”
“This standard was under review due to investigation, and was found to be deficient. The operation has not given the incident to the child's parent.”
“This standard was under review due to an investigation, and was found to be defiecient. The three children tested positve for COVID-19 and the operation notified the local health department and did not notify licensing.”
