Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The smoke detector in the kitchen, which is a room used by children, was beeping.”
“There was a bottle of Clorox wipes on the floor within reach of children. This was corrected during the inspection when the wipes were moved.”
“Two of the smoke detectors in the home were beeping and needed new batteries. One of the smoke detector's batteries was changed during the inspection.”
“The primary caregiver did not have any trainings for the 2024-2025 training year.”
“The permit renewal letter and employee list were not posted.”
“Out of four children's files reviewed, one child was missing an emergency contact, two were missing a parent's signature agreeing to the parent handbook, one was missing a health statement, two were missing immunization records, one was missing a medication administration log, and all four were missing sign-in and -out logs.”
“The CPSC form was last signed 5/15/24. This was corrected during the inspection when the caregiver signed the form verifying that she has reviewed the recall list.”
“There was a bottle of Clorox wipes on a counter within reach of children. This was corrected during the inspection when the bottle was moved to the top of the fridge.”
“One household member who was not counted in ratio was missing documentation of orientation.”
“A dog in the home had a rabies vaccination that expired in February 2025.”
“There was no fire drill recorded in February. This was corrected during the inspection when a fire drill was done and the children were able to exit the home in under three minutes.”
“The primary caregiver's first aid and CPR training expired in August 2024.”
“The dates of the most recent fire drill and tests for fire extinguishers, carbon monoxide detectors, and smoke detectors were not recorded. This was corrected during the inspection when the drills and tests were recorded.”
“Out of four children's records reviewed, two did not have the admission date and four needed updated immunization records.”
“The children's product recall form had not been signed since 2022. This was corrected during the inspection when the primary caregiver signed a new form verifying that she reviewed the recall list.”
“The home's most recent licensing report was not posted.”
“The first aid kit did not have a guide to first aid.”
“A cat in the home did not have any vaccination records available for review.”
“There were no infant feeding instructions for an infant who was still on formula.”
“During the inspection, an infant was watching television. This was corrected during the inspection when the primary caregiver was informed that children under two cannot watch television and the television was turned off.”
“The primary caregiver did not have training certificates available for review for training in SIDS and Shaken Baby Syndrome, early childhood brain development, abuse and neglect, emergency preparedness, communicable diseases, administering medication, preventing emergencies due to allergic reactions, understanding building and physical premises safety, or handling hazardous materials.”
“During the inspection the provider did not have documentation of current vaccinations for pets in the home.”
“During the inspection it was found that the primary caregiver CPR/First Aid certificate expired in December 2020.”
