Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“A total of two vehicles were observed. One of the two vehicle emergency break light was not working properly.”
“The operation is being cited by assessment because the operation failed to report the number of employees left the childcare center by the previous calendar year 2024.”
