Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During an investigation it was determined food was served after falling on the floor.”
“During an investigation it was determined a child under 18 months was grabbed, pulled, and then picked up by one arm as a form of guidance.”
“One child with diagnosed food allergies did not have an emergency plan signed by parent and health care professional.”
“During an investigation it was determined a two-year-old child was using a bike recommended for ages three to seven years-old. Bike did not have handlebar covers as required by manufacture.”
“Two out of ten employee files did not have a notarized affidavit to review at time of inspection.”
“Medication listed on two food allergy emergency plan were not available at the operation. One medication required on a food allergy emergency plan was observed as expired at time of inspection and not labeled with the date brought to operation.”
“Three out of eight up to date infant feeding instructions were not available for review at time of inspection.”
“A caregiver lifted a child off the floor by their hand which resulted in a minor injury to the child's left arm.”
“The operation failed to report within 48 hours a positive case COVID.”
“One child in care did not have an immunization schedule on file.”
“One child did not have a current immunization record on file.”
