Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The floors were observed to be in disrepair.”
“The fence was observed with broken planks and protruding nails.”
“The last fire inspection was conducted on 8/28/2025.”
“One caregiver had expired CPR/First Aid.”
“The operation had two pieces of non-stationary equipment with overlapping use zones. This was correct at the inspection when the equipment was moved to meet the required 12 ft of use zone space between them.”
“The operation did not have the required amount of loose-fill (6 inches) around the rocking/bouncing or sliding equipment.”
“Three children with diagnosed allergies did not have a special needs statement aligning with the diagnosis.”
“The operation's director did not use good judgment by having a caregiver present at the operation caring for children with a communicable disease.”
“Two children with diagnosed food allergy checked on their admission form did not have a food allergy on file at the time of inspection.”
“The director did not have all the required information on training certificates.”
“The director did not have transportation training at the time of inspection.”
“One caregiver did not have current pediatric first aid training.”
“One child did not have updated immunizations in their file at the time of inspection.”
