Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A caregiver was observed callng a child a crybaby.”
“Infant feeding instructions were not available for1 of the 3 bottle-fed infants in the classroom. This was corrected at inspection when the parent came to the center and completed the document.”
“A caregiver was observed turning on the tv for a group of children which included children under 2 years of age.”
“A caregiver was observed turning on the tv for children to watch while eating breakfast.”
“A child younger than 18 months old was observed to be in a group with older children, including Pre-K aged children.”
“This standard was tasked as part of an investigation and found deficient. There is personnel paperwork that indicates an individual is not qualified to maintain a role of caregiver at the operation. This individual was assigned caregiver responsibilities on multiple occasions.”
“This standard was tasked as part of an investigation and found deficient. A caregiver did not meet caregiver qualifications.”
“This standard was tasked as part of an investigation and found deficient. The infant classroom was over ratio by one for one caregiver.”
“This standard was tasked as part of an investigation and found deficient. The operation has not notified families of less serious injuries (ex: scratches and bruises) at pick up.”
“This standard was tasked as part of an investigation and found deficient. The owner/director does not have record of attendance.”
“One caregiver was seen sanitizing equipment and vaccuming while three children were present in the classroom.”
“One of the six cribs used in the infant classroom was recalled in January 2010. At the time of inspection, an infant was sleeping in the crib. This was corrected at inspection when the crib was removed and disposed of. The operation does not need the crib for the time being due to the enrollment in the classroom.”
“One of the two outdoor gates were locked and the employee did not have access to a key to unlock it in case of an emergency. This was corrected at inspection when a copy of the key was given to the caregiver and added in the list of things to bring outside with children.”
“The operational policies were missing section 18 subsections A, B, C, D, and F.”
“One infant was observed sleeping in a swing with a blanket. This was corrected at inspection when the child was moved to a bare crib.”
“One classroom was not being supervised during naptime.”
“Infants hands were not washed after diaper changes.”
“A caregiver was observed not washing their hands between each diaper change.”
“The operation did not inform Child Care Licensing of children being placed at risk within two days of the incident occurring.”
