Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A classroom was over in ratio during the inspection. The ratio was 18 children to 1 caregiver and the caregiver can only have 15 children based on the specified age group of 3 year olds.”
“1 child's file was missing an immunization record and 2 childrens files were missing a completed health statement.”
“The following documentation was requested and was not made available to review by Child Care Regulation. 1. Completed Incident Illness Report 2. Documentation of the Sign in and out Log for February 25th, 2026 3. Documentation of a caregiver's CPR and First Aid Training Certificate”
“A caregiver did not know the correct number of children they were responsible for.”
“A food allergy emergency plan was missing for 1 child.”
“The operation did not have a completed health statement on file for 1 of 4 children's files evaluated.”
“The nap mats were not labeled during the inspection.”
“There was insufficient walking space inbetween cribs during the inspection. Note - The operation rearranged the cribs during the inspection.”
“The annual fire inspection has not been conducted since September 13th, 2024.”
“The paint on the walls in the Pre-School 1A classroom was chipped”
“A diaper changing mat was torn and cannot be cleaned properly.”
“Based on the information obtained, a caregiver did not practice good judgment. The caregiver did not thoroughly clean a child while diaper changing.”
“During the walkthrough, I observed a classroom with 24 children between the ages of 4 and 5 years of age. Note: This was corrected when 9 children from the classroom were removed and taken with another caregiver.”
“During the inspection the operation did not have documentation of the updated fire inspection report available to review.”
“During the monitoring the center did not have a current health inspection available for review.”
“Two out of five files was missing training in the following: Child growth and development, Guidance and discipline, Emergency preparedness and Preventing and responding to emergencies due to food or an allergic reaction.”
