Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Outdoor activity space had a trailer parked with items going to storage. A pile of branches was in the activity space. Metal chain link pieces from the new fence were located on the ground in the activity space.”
“Swing sets did not have the required forward and back use zones. The set requires 140 inches front to back, starting from where the swing seats are at rest.”
“The operation's daycare building is separate from the residence building. The daycare building does not have the required two exits. Windows that may count as a second exit for emergencies do not appear accessible enough for children and caregivers to exit safely and quickly.”
“Some areas of the outdoor space accessible to children had perimeter fencing that was under 4 feet high. The areas on the other sides of the fence led to other property, including property with 2 large dogs that appeared possibly aggressive. The home is also within proximity to a lake.”
“The operation has not verified liability insurance coverage. Verification of liability insurance was due by 11/12/2024.”
“Several plugs were observed uncovered.”
“The primary caregiver could not locate her file, including her annual training records, during today's inspection.”
“In reviewing 4 of 8 children s records, three were lacking immunization records.”
“The household member did not have orientation on file for review.”
“In reviewing 4 of 8 children s records, the following were lacking: - One date of admission - Three emergency contact information - Three emergency car authorization - Two doctor s information”
“In reviewing 4 of 8 children s records, all four were lacking completed healthcare statements.”
“The written operational policies were lacking: (2)Illness and exclusion criteria; (8)Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan). Your policies must include: (A)The duration of physical activity at your home, both indoor and outdoor; (B)The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities ; and (C) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor time.”
