Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of five staff files evaluated did not have training available for review for Responding to Food Allergies, Handling Hazardous Materials, or Administering Medications”
“Two infants did not have current infant feeding schedules.”
“A caregiver counted in ratio in the school age room did not have Eligible results to be counted in ratio.”
“Based on information gathered, it was determined that caregiver used profane language toward a child.”
“Based on information gathered, it was determined that caregiver roughly grabbed child by the arm.”
“Based on information gathered during the course of the investigation, it was determined that caregiver engaged in inappropriate interaction with the child when she kicked the chair in which the child was sitting on.”
“Based on the evidence gathered, the use zone around the climbing playset was not free from hazards when a child fell off the structure and landed on a kitchen pan that was left behind by another child while playing on the playset.”
“Supervision was not appropriate when a three year old child walked out of the classroom to the water fountain located outside the classroom. Caregiver was not aware that the child had left the classroom to drink water.”
“Two fire extinguisher have not been serviced. The fire extinguishers were last serviced August 2021.”
“Three out of the five staff files reviewed did not include proof of education.”
“A list of the children's food allergies was not observed posted in one classroom and in the kitchen where food is prepared. Note: This was corrected at inspection when the Director posted the food allergies.”
“Three out of the five staff files reviewed did not include proof of education.”
“The three year old classroom was observed out of ratio with one caregiver to 18 students. The actual ratio is one to 15. Note: This was corrected at inspection as a caregiver stepped into the classroom.”
