Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“A child in the school-age classroom and a child in the Pre-K classroom were observed napping in front of the exit door. The caregivers corrected the deficiency during the inspection by moving the children to a save area in the classroom.”
“Two drivers that transport children do not have current transportation training.”
“Three caregivers do not have current pediatric first aid training.”
“Three caregivers do not have current pediatric CPR training.”
“Training certificates were not available for two caregivers.”
“An infant was observed utalizing equipment that have been recalled by the Consumer Safety Product Commission. The caregiver immediately removed the infant out of the equipment and placed the infant on the floor to engage in floor time. Management disgarding the equipment during the inspection.”
“The operation does not have a current annual fire inspection. The last fire inspection was conducted December 2022.”
“This standard was found to be deficient as 5 children did not have their medical authorization information completed in their file.”
“The operation was not able to provide a current Fire Inspection during the walk through of the facility.”
“There was a rocking chair in front of the fire exit in the second infant classroom. Picture was taken.”
“A caregiver was observed in charge of 10 toddlers ranging from 18 to 24 months. The ratio for toddlers is 9 to 1.”
“The non-absorbent seating chairs in the school-age classroom are not maintained. The non-absorbent material is torned and ripped exposing the fabric which can pose a risk of children spreading germs.”
“A caregiver was observed in charge of 5 infants ranging from 8 weeks to 12 months. The ratio for infants is 4 to 1. The deficiency was corrected duirng the inspection when the person in charge transitioned one infant out of the classroom to go on a buggy ride in the building with 2 other infants.”
“The operation has not completed the annual certification for recalled products.”
“Four persons need transportation training.”
“Training has been completed but it has not been documented on a training certificate.”
