Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During review of record keeping 1 out of 10 files evaluated did not include a completed food allergy plan as required.”
“During review of infants 2 out of 8 infants present not yet eating table food did not have current feeding instructions as required.”
“A caregiver was found to have left a 9-month-old infant in a classroom unsupervised.”
“During the walkthrough I observed a 3 month old asleep in a restrictive device for approximately 10 minutes. This was corrected when the caregiver removed the child and placed the child in the crib.”
“During the walkthrough I observed a 3 month old asleep in a bouncer with a blanket covering her face and tucked around her body. This was corrected when the caregiver removed the blanket.”
“It was determined that 2 year old with special needs was left alone on the playground when the class went inside.”
“The director was not in compliance with ensuring supervision is being maintained with children in care. The operation was previously cited on 8/17/2021 and 6/15/2021 for a child being left unsupervised.”
“During review of Fire Safety although the operation conducted drills for the month of November it was not documented. This was corrected when the director documented the drills conducted.”
“During the inspection I observed a child not washing their hands after getting a diaper changed. After addressed the caregiver instructed the child wash their hands.”
“A child was left on the playground for a short amount of time when the classroom went inside. The child was discovered by another caregiver.”
