Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The operations director was not present for the inspcetion and does not come 30 hours a week or 75% of the time.”
“A caregiver was observed to have direct access to children without an active background check.”
“A caregiver did not demonstrate good judgment while holding an infant beside a hot bottle warmer.”
“A caregiver held an infant in an unsafe area of the room that included water up to 202 degrees Fahrenheit and electrical cords. The situation presented a situation that could not allow time to intervene should an accident occur. A bottle warmer was knocked over, and the infant received 2nd degree burns.”
“The operation did not have the fire inspection but did have an invoice that they paid forn inspetion in June of 2024. The operation reached out to the local fire department at inspection via email to get a copy of the inspection.”
“Two of the children's records reviewed did not have immunizations on file.”
“The operation waiting an hour and a half before notifying the parent the child was left unsupervised.”
“Staff did not use good judgment in following the operations policy for counting children during transitions.”
“Caregivers left a child outside on the playground unsupervised for approximately 15 minutes”
“This standard was evaluated as part of a DFPS Investigation and found to be deficient.”
“Two of the school aged children were observed in the infant Room 2 class.”
“The operations sanitation inspection expired 7/7/2023.”
“The operations liability insurance expired on 7/10/2024 and did not have an updated policy for review.”
“Two of the tricycles on the playground had broken or cracked plastic seats.”
“The operation has not reviewed the CPSC website since 6/1/2023 according to documentation.”
“Last documented fire drill 6/15/2022. The operation stated they conducted a fire drill at the end of August but did not document the date and has not conducted a fire drill for the month of September.”
“Three infants in the infant classroom had outdated feeding instructions.”
