Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“An affidavit observed for exemption from immunization for reason of conscience was not notarized, and/or signed. The parent signature was dated December 7, 2021.”
“Two out of five children's records observed did not include a health statement. Both records observed included less than 1 year of enrollment.”
“One out of five children's file did not contain the address for the emergency contacted listed.”
“The operational polices did not include information pertaining to the promotion of indoor, and/or outdoor activities. The operational polices did not address alternative physical activities either in case of extreme weather conditions. The operational policies did not address recommended clothing, and/or footwear when children are engaged in outdoor physical activities at the center.”
