Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One of the vehicle's fire extinguishers lacked a current service tag, the most recent inspection tag dated back to January 2025. The fire extinguisher in one of the transportation vehicles was unsecured because its bracket was broken.”
“The rearview mirrors of the two transportation vehicles were blurry, and one of the vehicles had worn-out tires. Both transportation vehicles had trash, including plastic wrappers, crayons, and other items, on their floor and slot compartments.”
“The operation does not have a current fire inspection. The last fire inspection was conducted on 4/04/25”
“The sign in and out log was not current. There were 13 children present and only 7 children were signed in. Corrected at Inspection: All children were signed in during the inspection.”
“A caregiver did not have a current CPR training.”
“The childcare center failed to notify Child Care Regulation of an incident involving a child who required medical treatment.”
“The food allergy emergency plan for two children was not signed by the parent. One food allergy plan did not indicate the symptoms and the steps to take if the child has an allergic reaction.”
“Three infants did not have an infant feeding instruction form.”

“There was no activity plan for the Prek group. The activity plan for the toddler group was not dated. Note: A new activity plan for the week was posted for the toddler group.”
“Loose fill surfacing (pea gravel) was not maintained at the proper depth for a play structure in the outdoor play area. In addition, the depth of the pea gravel was not marked.”
“A 4-month-old infant was asleep in a restrictive device, swing. Note: The caregiver removed the infant from the swing and placed them in the crib.”
“Two classrooms were observed with at least 4 ceiling tiles with holes and water stains.”
“The sign in and out log was not maintained. Corrected at Inspection: The times and initials were placed for those who had not been sign in.”
