Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three of the four children files didn't have "not applicable" for special needs. One of the four children is missing page 2 of the child's enrollment form and signed operational policy.”
“During snack time the children were provided vanilla Oreos.”
“No documentation of the severe weather was available on the emergency practice.”
“In both of the toddler classrooms two trashcans accessible to children were observed with no lids that contained food in them. NOTE: This was corrected when the trash was taken out.”
“Three of the four staff did not have Form 2947 that reviews the orientation.”
“The following written statements for Promoting of indoor and outdoor physical activity were not available for review.”
“The first aid kit was observed missing the guide to First aid and emergency care. NOTE: Corrected at inspection.”
“In the four of the four children's admission forms the healthcare information was missing the addresses to the hospital/physician listed.”
“The Children's Products Certification Form 2885 was unavailable for review. NOTE: This was corrected at inspection.”
“Although there is a trashcan with a lid used for diapers, several diapers were observed in another trashcan without a lid accessible to children. NOTE: The toddler caregiver changed out the trashcan and started using the other trashcan with a lid used for diapers.”
“A toddler caregiver was observed not changing gloves and washing hands between each diaper change. NOTE: This was corrected as several diaper changes were observed as gloves were changed.”
“The facilitys operational policy did not specify on water play, gang free, and how to access Licensing inspection.”
“The last fire inspection on file was conducted on September 17, 2020.”
“Director did not have the training available for 2020-2021 available for review.”
“Four of the four staff were missing the food allergy training.”
“Two of the four children did not have an updated immunization record.”
