Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One child with a food allergy is missing the action plan.”
“Three staff were missing a notarized affidavit.”
“Ten staff files were missing that they have not signed off on receiving the operational policies.”
“The operation did not check the bottles to make sure the label stayed on the bottle preventing the infant from choking.”
“The operation did not use good judgement when they left details out when telling the parents about the incident.”
“The operation did not report to Licensing that a child choked and threw up blood.”
“The operations' loose fill is decomposed throughout the use zones and did not meet the required 6 inches of loose fill.”
“The operation has not been maintaining menus for the last three months.”
“The operation has mediation authorization forms that signed by parents that exceed label directions and authorization forms for mediation not to be used by "said" age group.”
“Two caregivers at the operation do not have CPR/First Aid that meet guidelines.”
“The operation has been administering medication that exceed label directions and do not have a physicians note.”
“Two caregivers that have been employeed for over 90 days do not have the required 24 hours pre-service traning.”
“The operation does not have a food allergy plan for one child in care with an known food allergy.”
“On 02/02/2022, the mini-tots classroom was out of ratio for forty-five minutes during naptime. There were nine children in care and one caregiver in charge. The specified age group was twelve to seventeen months.”
“A substitute caregiver hired from a temporary employment agency, did not wash an infant's hands after changing their diaper.”

“An infant was strapped in an infant swing sleeping at naptime.”
