Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The standard is evaluated for a standard investigation and is not found compliant. A child made an outcry to a parent that a caregiver pinched them which poses a risk to bodily harm.”
“This standard was evaluated as part of an investigation by the Department of Family and Protective Services and was found to be deficient. Multiple testimonies indicated the caregiver used verbally harsh and abusive language. Witnesses reported the caregiver used derogatory and demeaning language, creating an emotionally harmful environment.”
“This standard was evaluated as part of an investigation by the Department of Family and Protective Services and was found to be deficient. The caregiver threw away food before children were done eating, forced a child to use the bathroom, and yanked a mat from underneath a child during nap time. Specific incidents include discarding a child's food for talking too much, forcing bathroom use against their will, and disrupting naps by pulling away mats, causing children to fall. These actions were confirmed by multiple interviews with caregivers who observed these inappropriate disciplinary measures.”
“This standard was evaluated as part of an investigation by the Department of Family and Protective Services and was found to be deficient. The caregiver's actions were aggressive and included grabbing and verbal threats. Witnesses reported the caregiver yanked a child's jacket, pulled a child by the wrist, and used threats to control behavior. Multiple interviews with caregivers confirmed these actions, indicating a pattern of aggressive behavior.”
“A child that has been enrolled since 2020 does not have a health statement completed by a health care professional.”
“As a result of an investigation it was determined that this standard is deficient. A caregiver did not use appropriate redirection with a child when the child moved into an area behind the table where they were not supposed to be.”

“While evaluating employee files, I observed that one employee did not have a current copy of CPR training in their file. The director stated the employee has completed the training but has not yet received the card.”
“I observed while reviewing personnel records that three operation employees were missing the signed employee affidavit form their employee files.”
