Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“One out of eight employee files is missing the emergency preparedness training.”
“The caregiver in charge has conditions prohibiting them from being left in charge of the operation.”
“Fire inspection was last conducted on October 18, 2022, and is expired.”
“The emergency contact's name and phone number was not in the transportation binder for all children tranportated by the operation.”
“Based on investigation findings, children's soiled diapers are not being changed when needed.”
“Based on investigation findings the operation is not allowing parents to walk into the center. Drop off and pick up procedures are done at the front door.”
“Children were not counted as they exited the transportation vehicle and entered the center. This resulted in a child not being accounted for.”
“The caregiver who transported children did not use the alarm as intended. The alarm was deactivated from the outside, instead of a walk-through being completed and the alarm then being deactivated from the inside of the vehicle.”
“Neglectful supervision occurred when a six-year-old child was left on the center's van for approximately 45 minutes.”
“Children ages 4yrs-12yrs did not wash their hands with soap and water before eating their lunch.”
“A caregiver responsible for the 4yr-12yr olds was observed on their cell phone while caring for the children. The caregiver observed was on Facetime.”
“A caregiver age 17 years old and does not have a high school diploma was observed caring for 20 children (ages4yr-12yr) and left in charge with sole responsibility of the group of children.”
“Multiple children between the ages of 4yr-12yr were observed playing on their cellphones during the inspection. Note: The caregiver prompted the children to put away their cellphones.”
“A caregiver responsible for the 4yr-12yr olds did not know the number of children they were caring for. Another caregiver responsible for the 1yr-2yr olds did not know the number of children they were caring for.”
“The operation falied to submit a renewal for a person's background check no later than 2yrs from the date last submitted. The background check was last submitted on 1/11/2020.”
“The diaper changing pad in the infant room contained rips/tears in it.”
“During the investigation inspection, it was determined a staff member tested positive for Covid 19 and the operation did not report the case to Child Care Regulation.”
“A caregiver was observed at the operation caring for 4 children (ages 6m-1y) without an initial background check. Another caregiver was observed at the operation caring for 10 children (ages 1y-2y) without an initial background check.”
