Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Children are picked up at the ISD, and taken outside for a snack.”
“Two children's files were missing immunization records.”
“The personnel and training records were not all available to review at the operation today.”
“Three infants were missing updated infant feeding schedules.”
“One caregiver was alone in a room with 9 infants and toddlers. The specified age group was 12 - 17 months.”
“During the follow up inspection, it was found this citation was partially fixed. There was still one urinal that was out of order and a sink out of order.”
“There was one urinal that was out of order, and there was also a sink out of order. Additionally there was a toilet that was leaking water, every time it was flushed. Also there is a water fountain that recently had a leak as well.”
“There was an insufficient amount of loose fill surrounding the swings and rocking equipment in the outdoor play space. There was dirt and small rocks that were showing under the loose fill.”
“There was a child on the diaper changing table, while the caregiver was on the other side of the classroom. Note: The caregiver walked over to the diaper changing table and started assisting the child on the diaper changing table.”
“A caregiver was observed to not wash her hands after changing a child's diaper. This was corrected when the caregiver washed their hands.”
“The child's medication log record was reviewed during inspection. On the medication label the amount of Methyin is 10ml dated 4/2/2022, the medication log record is documented child received Methyin 5ml 1/26/2022 through 5/5/2022. The correct amount of medication 10ml was not administered until 5/6/2022 with the administrator's signature, date and time.”
“During the walk through of the infant room none of the infant bottles observed were labeled with the children's names.”
“During the walk through in the infant room a caregiver did not wash an infants hands after completing a diaper change.”
