Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Safety drills have not been documented since August 2025.”
“The permit renewal notice was not posted. This was corrected at inspection when the permit renewal notice was printed and posted.”
“The first aid kits inside the operation were lacking tweezers. The first aid kit on the van was lacking a thermometer and scissors.”
“The children's products certification form had not been certified since May 2023. This was corrected at inspection when the form was signed and certified.”
“The transportation van only had 1 booster seat and they transported 7 Kinder children.”
“One of the booster seats in the van was manufactured 03/2015.”
“Three out of five children files reviewed did not have the date of admission.”
“The list of employees was not posted.”
“The caregiver did not practice good judgment when a child was released without obtaining proper identification of the individual.”
“The permit holder failed to follow their own policy for the release of children.”
“The caregiver did not know the name of the child released and therefore the wrong child was released to CPS.”
“The operation did not follow their release policy when a child was released to an individual that was not verified by the release forms or parent. No ID was obtained by the operation.”
“An incident report was not completed or provided to the parent after the incident occurred.”
“The operation lacked a current fire inspection.”
“One van lacked a child safety alarm.”
“The operation lacked a current health inspection.”
“One out of four staff records lacked an affidavit”
