Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“This standard was evaluated in regard to a report. The incident report was not shared and signed by the parent of the injured child within 48 hours of the injury. This deficiency is corrected - the parent has signed the incident report.”
“One medication was past the expiration date.”
“A parent's signature was missing on a food allergy emergency plan.”
“Two personnel files were missing emergency preparedness training certificates.”
“One child's file was missing a current immunization record.”
“An individual who did not have the background check eligibility determination, was in the presence of children in care. The individual was performing caregiver duties at the operation and had direct access to children.”
“The director did not have a record of daily hours worked.”
“One child's file did not have a current immunization record.”
“One of the reviewed children's files were missing the healthcare professional statement.”
“Four of the reviewed children's files were missing detailed special care statements.”
“The reviewed children's files were missing the required parents rights forms.”
“The director does not have daily attendance records on file.”
“Staff CPR/First aid training was not on file at time of inspection.”
“Four child records were evaluated. 2 health statements were not on file.”
“Four child records were evaluated. 2 shot records needed to be updated; 1 shot record was not on file”
“Notarized Employment Affidavits were not on file for staff.”
