Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“It was determined that the operation failed to report a high-risk situation to Licensing as required.”
“At the time of inspection none of the staff had current food allergy response training.”
“At the time of inspection the operation did not have the name and address of an alternate shelter for emergency evacuation relocation.”
“At the time of inspection one personnel file was not available for review.”
“Based on information obtained through investigation, it was found a caregiver walked away from water spilled out of a bottle warmer to get paper towels. When doing this an infant in care went to the water and touched it, causing injuries to their hand and arm.”
“Based on information obtained through investigation, it was found that caregivers in the operation heard an infant crying and came to the classroom to comfort him while his assigned caregiver was present multiple times.”
“Based on information obtained through investigation, it was found a caregiver had withheld information about how a child was injured in care. This information was not provided to the operation until later the same day, after the caregiver had left and the child had been picked up and taken to seek medical care.”
“It was determined that the operation did not notify Licensing of a communicable disease in a timely manner.”
“At the time of inspection the operation did not have a current health inspection conducted for 2024.”
“At the time of inspection the operation's annual fire inspection had not been conducted. The last fire inspection was dated 8/30/22.”
“During investigation, it was determined that a caregiver alone with a group of children did not have pediatric first aid training at the time of a playground injury.”
“The operation had a total of 10 children in care with 1 caregiver during nap time. There were 2 children under 17 months old. There were 8 children between 18 months and 4 years old. Based on ratio standards for the ages present for 12 or fewer children in care, the operation was out of ratio by 4 children.”
“One child's file did not have updated immunization records.”
