Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Three children did not have a doctors signature or stamp on their immunization record.”
“The childrens product safety form has not been updated since 7/2024.”
“Two staff did not have hazardous material training.”
“An infant was heard crying for a period of 6 minutes and neither caregiver in the classroom responded to the child's needs. The caregivers were observed speaking with one another and not engaged in caregiver responsibilities. This was corrected when the caregiver picked the child up, wiped the child's face and consoled the child.”
“Ten personnel records were reviewed. The following were missing: Five out ten files were missing orientation documentation Fiive out of ten files were missing documentation of education requirments. Three out of ten files were missing annual training hours. One staff was missing 13 hours, the second staff was missing 12 hours, and the third staff was missing 24 hours. Four out of ten files were missing preservice training.”
“Observed five wild baby rabbits inside a teepee play structure on the playground that is accessible to children in care. This was corrected by the rabbits being removed from the playground.”
“There was no documentation of emergency prepardness drills available for review.”
“The director did not have valid CPR certificate becuase it was conducted only online.”
“During the inspection a caregiver was observed in the classroom responsible for children without a cleared background check. This was corrected by the caregiver leaving the premises.”
“Out of six personnel records reviewed, the following was observed: Five records were missing proof of orientation completion; Four records were missing proof of background checks; Two records were missing proof of SIDS/Shaken Baby Syndrome training; and, Two records were missing Proof of Education and a Photo ID.”
“Out of ten children's records reviewed, one record was missing an emergency contact address.”
“A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.”
