Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on investigation findings, caregivers were not actively supervising the children on the playground resulting in misuse of a piece of equipment that caused an injury.”
“The operation's ouch report did not provide the parent a complete report that contained all of the required information on the incident/illness form, 7239. Form 7239 was provided to the parent, and signature obtained 14 days after the accident occurred.”
“Parent Rights information was not found upon review of the Operational Policies.”
“One out of five child files evaluated was incomplete. The file was missing information regarding authorization for emergency medical attention, permission to be released, special needs and parent signature page.”
“Children did not wash hands before eating snack.”
“A fire drill was not recorded for the month of April.”
“A current health inspection or letter indicating an inspection is not required was not available.”
“The operation will use space along side of community programs being held during the day.”
“An emergency preparedness plan had not been created specific for the new location.”
“A fire inspection was not conducted for the program space.”
“7 out of 19 child files reviewed were either missing an address for their listed emergency contact person on their file or the addresses were incomplete.”
“The operation's fire drill log did not contain documentation showing the length of time it took to fully evacuate from the building.”
“3 out of 12 student files evaluated did not contain addresses for the listed emergency contact person on the enrollment form.”
“The operation's had a liability insurance document on file that expired 12/10/21. A current copy of the insurance was not available for review during the inspection.”
