Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The fire extinguishers were observed to not be within the proper fill level and were observed to have last been serviced in October of 2024,”
“There were to toys with broken pieces and a picnic table with cracks observed on the playground. The operation removed the toys during the inspection and the children will not have access to the playground with the cracked table. The table will be removed on 10/10/25 when the operation is closed.”
“It was determined the operation did not report an incident involving prohibited punishment, that placed a child at risk.”
“It was determined the operation did not inform a parent of a situation that placed their child at risk.”
“It was determined a caregiver placed a child in a dark bathroom with the door open.”
“Based on the DFPS investigation, it was determined that proper supervision was not maintained during the incident as caregivers were observed sitting down and not actively walking around the playground.”
“It was determined proper supervision was not maintained during the time of the incident. Caregivers were observed on video sitting down during outdoor play and were not properly walking around to intervene as necessary.”
“There were 2 caregivers with expired CPR certifications.”
“There was a caregiver with a provisional background check and the caregiver was observed supervising a group of children alone.”
“The gas inspection was last completed on 9/8/2020.”
“During the investigation inspection a caregiver wiped the noses of children and did not wash their hands until asked by Child Care Regulation to. Caregiver washed her hands after asked to.”
“During the investigation inspection it was observed that the boys bathroom in the pre k classroom had dried urine on and around it. This was corrected at inspection after it was cleaned.”
“An incident occurred where a child was hit and yelled at by a caregiver. This incident was caught on camera and the caregiver was fired.”
