Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“The guide was missing from the first aid kit.”
“The operation had not completed the grounds checklists since the week of May 13-17.”
“One out of 10 employee files evaluated had a CPR card that expired in June 2026.”
“There were two mop buckets found in two restrooms. This was corrected when the director removed the two mop buckets.”
“There was sufficient evidence to suggest that the operation does not always notify parents of less serious injuries.”
“One of the children did not have up to date immunization records.”
“A 12 month old infant was observed standing in a crib with a bottle. This was corrected when the caregiver took the bottle from the infant.”
“A two month old infant was observed with a bib, burp cloth, and toy in the crib. This was corrected when the caregiver removed the objects from the crib.”
“The fire extinguishers were not checked in 2024.”
“The operation's fire marshal inspection was not up to date.”
“Four of Ten Children's Records did not have immunization records.”
“Two of Ten Employee Records did not have the First Aid Component of the CPR/First Aid Training.”
“The fire drills did not have times of exit recorded with them.”
“Seven of ten children's records did not have complete emergency release information.”
“Eating utensils were seen in a cup on the edge of the diaper changing sink. This was corrected when the director instructed the caregivers to move the utensils and not store them there anymore.”
“Four of ten children's records did not have updated immunization records.”
“Three of ten children did not have complete emergency release authorization.”
“The most recent fire inspection was not posted and could not be produced upon request.”
“Nap mats were not labelled by name or number to indicate which child they belonged to.”
“The operation did not have the up-to-date annual carbon monoxide detector inspection documentation available.”
