Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Based on the information obtained, it was determined a child was unaccounted for and a caregiver was unable to locate the child. The child was located nappiing in the school age classroom and was alone for approximately 6 minutes.”
“There were 3 caregivers present and all the caregivers were observed supervising a group of children. There was not another caregiver observed in the building during naptime.”
“Based on the preponderance of information, it was determined the operation did not report allegations of prohibited punishment.”
“Based on the preponderance of evidence it was determined that a caregiver grabbed a child's hand roughly causing it to become red.”
“It was determined a caregiver grabbed a child by their upper arm.”
“A caregiver was observed wiping a child's nose and was observed to not have washed their hands afterwards.”
“A child was observed to have had their diaper change and the child did not have their hands washed afterwards.”
“Based on evidence obtained from the investigation it was determined a caregiver would lose her patience often and speak with a harsh tone to the children in care.”
“Based on the evidence obtained through the investigation, it was determined a caregiver would cry and become emotional in front of the children in care. The caregiver's behavior happened frequently and became concerning to other staff members.”
“Based on the preponderance of evidence, it was determined children were being forced to sleep during naptime.”
“It was determined that the Director was informed of two incidents where children were being forced to nap and the Director did not report to Child Care Regulation until the 2nd incident occurred.”
“It was determined a caregiver did not intervene when she witnessed another caregiver forcing children to sleep.”
