A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“During the inspection a 4 month old was being cared for in a room with a 4 year old.”
“During the inspection an infant was observed sitting in a restrictive device for over 15 minutes.”
“During my inspection both caregivers were not aware of who was in charge while the director was out.”
