Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Children's product certification form was last reviewed on 2023 which makes it expired. NOTE: This was corrected at inspection when the recall list was reviewed and the form was signed and updated.”
“One of the caregiver records being reviewed is missing 1 hour training for prevention, recognition, and reporting of child maltreatment required for the annual trainings.”
“During nap time the mats did not have enough space to walk between them. NOTE: This was corrected at inspection when caregiver rearranged the mats giving space between them for caregivers to walk.”
“The ceiling air vent was dusty on the outside area. NOTE: This was corrected at inspection when the vent was cleaned and the air filter was replaced with a new one. In the toddler restroom there was a can of hairspray that stated keep out of reach of children. NOTE: This was corrected at inspection when caregiver removed the hairspray from under the changing area making it inaccessible to the children.”
“Cribs were labeled with letters but there was no list as to who the crib belongs to. NOTE: This was corrected at inspection when caregiver posted a list on the wall with the letter and the child who it belonged to.”
“Staff files were reviewed 3 out of the 5 staff files were missing photo identification. NOTE: This was corrected at inspection when caregiver made a copy of the 3 employees who were missing the photo identification in the file.”
“There was an ointment in the infant room that was not labeled with the child name. NOTE: This was corrected at inspection when caregiver put the name of the child on the ointment.”
“During application inspection the staff files were evaluated. The employee personnel records were missing for the employees.”
