A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.

“Operation did not have a completed monthly maintenance checklist available for review for 10/01/26 through 10/18/26. This was corrected at inspection, when the director located a maintenance checklist dated 10/19/26 - 10/22/26 at the end of the inspection after the director arrived at the operation.”
