Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“During the investigation it was found that children who are too young and small to sit in a vehicle without a booster/car seat were transported without the appropriate seats. It was also found that more children were transported than the seatbelts available in the vehicle.”
“During the investigation it was learned that children who are not yet 12 years old were transported in the front seat of the vehicle.”
“The most recent fire drill was completed on 09/11/2025.”
“One caregiver was supervising children in 2 rooms. A second caregiver was supervising children in another 2 rooms.”
“The most recent fire inspection was completed on 10/5/2022.”
“The most recent health inspection was completed on 10/31/2022.”
“Two of four files reviewed were missing acknowledgement of orientation. One was completed during the inspection.”
“The transportation vehicle transports 8 people including the driver. There is no child safety alarm in the vehicle.”
“A toddler child in care was sleeping on an oversize stuffed animal.”
“A 10 month old infant who is not yet walking independently is sleeping in a map.”
“The staff records were incomplete during the inspection.”
“The children were divided into two groups for naptime. One in each classroom. There was one caregiver supervising both groups.”
“There was more than 18 months between the youngest and oldest children in care in a classroom. The youngest child in the group was 8 months old. The oldest child was 3 years old.”
“A five month old was left unsupervised in the infant room. The center has an open concept. The child can be heard from other areas in the center but there was no caregiver in charge of the child.”
“The interest centers have not been set up.”
