Violations
Text reproduced verbatim · Texas Health and Human Services Commission, Child Care RegulationNearly every licensed provider accumulates some. A missing signature and an unlocked medicine cabinet are both “violations”, and they are not the same thing. Look at the class, the pattern over time and how quickly it was corrected — not the count alone.
Class is our grouping of the state’s own weights, reviewed per state. The agency’s own label is always shown beside it.
“Two children's files did not have current immunization records.”
“One child's admission form did not have emergency medical authorization statement completed.”
“Based on information obtained it was determined that the operation did not make adequate adjustments to supervision in the classroom to prevent children from becoming too rambunctious and causing injuries.”
“At the time of the inspection the operation had not had a gas inspection completed in the required time frame.”
“A caregiver sprayed a child with soap and water while cleaning the tables.”
“At follow up it was discussed that caregivers are able to use a child lock on the closet doors in the classrooms and it was discussed with caregivers that these should be closed so children do not enter.”
“This was evaluated as part of a DFPS investigation and found deficient. Based on information provided a caregiver could not maintain visual awareness or ability to intervene when a child was shut in a dark closet inside the classroom. The room is used to store nap mats and other class items.”
“This was evaluated as part of a DFPS investigation and found deficient. Based on information provided the operation did not create an incident report for an incident where a child was found in a closet inside of a classroom.”

“4 of the 10 children's records reviewed did not have updated immunization records.”
“At the time of the inspection 4 of the 10 employee records required the 2912 form and it was not signed or present in the employee's file.”

There is a section of the playground border that has a crack that creates a concern for pinching or a child getting their finger stuck in this.”
“At the time of the inspection the loose fill around the preschool playground structure was not maintained at a depth of six inches. Dirt underneath fill was visible without effort of moving it and grass growth through the fill was visible.”
